29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. (a) On or about October 10, 2024, and at times prior, at 901 North 9th Street, Milwaukee, WI 53203, the employer had not trained employees by a competent person to operate the OZ Lifting product 1-ton gear trolley (Model: #0Z1GBT) prior to the use of the trolley system on the suspended scaffolding.
