29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: (a) Basement and blender floor; On or about July 24, 2024, and dates prior, the employer did not ensure that each employee affixed their own lockout lock on isolation points for hazardous energy sources or group lockout device for the 6-ton mixer, blender, and AG-10 auger while they performed cleaning operations.
