29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Main Building, Rotary Die Cutter Line - Employees performed maintenance and servicing work, such as unjamming, on the AG Stacker, model AGS2000, serial number 05-060043, which included standing on a floor level powered (but not moving) roller conveyor, and did not use lockout/tagout, exposing employees to hazards including, but not limited to, caught-in and struck-by, on or about March 12, 2024. (b) Main Building, Finishing Department - Employees performed servicing and maintenance activities, including, but not limited to, set up, on the Post 84-inch folder/gluer, and did not use lockout/tagout, exposing employees to caught-in and struck-by hazards, on or about March 12, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
