29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: At the worksite located at 1315 North 2nd Street Clinton, MO: On or prior to January 29, 2024, employees were exposed to respiratory, atmospheric, and explosive hazards. Employees worked at a grain elevator with twelve grain bins and two receiving pits and were required to enter these as part of job duties without a written and implemented permit-required confined space program.
