29 CFR 1926.1431(f)(3)(i): Materials and tools were not secured to prevent displacement: (a)Northland Concrete and Masonry Company, LLC at 1508 West 22nd Street, Sioux Falls, SD 57105: On or about and at times prior to May 25, 2023, the exposing employer did not ensure employees were protected from falls, crushing, and struck-by hazards in that, formwork, metal box of ties, and associated equipment being hoisted on the Doka work platform were not secured to the platform. This condition exposed employees to falls, crushing, and struck-by hazards.
