29 CFR 1910.119(e)(3)(v): The Process Hazard Analysis did not address facility siting: (a) Protein for Pets Opco, LLC, dba All American Pet Proteins, at 259 30th Street, Greeley, CO: On and preceding 9/3/2021, the Process Hazard Analysis did not address facility siting. Employees operated and maintained an ammonia refrigeration system. The refrigeration system contained approximately 12,700 pounds of ammonia. Production lines in the facility utilized liquid nitrogen. A large liquid nitrogen pressure vessel was located onsite. The production lines containing blenders with nitrogen feed lines were adjacent to the plate freezers, which utilized the ammonia refrigeration system. The Process Hazard Analysis did not address the impact of a release of ammonia on the liquid nitrogen system, nor did the Process Hazard Analysis address the impact of a release of liquid nitrogen upon the ammonia refrigeration system. This condition exposed employees to an ammonia hazard.
