29 CFR 1910.179(j)(2)(iii):Monthly inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, were not performed: At a facility located at 2221 North Belcrest, Springfield, MO 65806: The employer failed to protect employees from struck-by or crushing hazards. This was most recently demonstrated on or about May 4, 2021, and at times prior thereto when employees were permitted to operate an overhead crane in the facility that had a crane hook that was damaged and missing a safety clasp.
