29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: a. Main Working Level, Paper Machine #9- Employees were not protected from a trip hazard where a floor hole, approximately 13 inches wide, on the drive side near the Yankee dryer was not covered. b. Basement, Paper Machine #9- Employees were not protected from a trip hazard where a floor hole in the catwalk around the hydro pulper was not covered.
