29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves. a. Located at the Slitter 1 line: On or about March 1, 2017 and at times prior, employees were exposed to a caught in hazard while operating, observing, and inspecting material being processed on the line as a result of two unguarded, approximately two and a half inches in diameter, arbor shaft ends projecting out the side of the slitter frame approximately two and a half inches. b. Located at the Slitter 1 line: On or about February 14, 2017 and at times prior, employees were exposed to a caught in hazard while operating, observing, and inspecting material being processed on the line as a result an unguarded, approximately three and a half inch shaft projecting out the side of the chopper machine approximately thirty four inches.
