29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag shall be placed on each disconnecting means used to deenergize circuits and equipment on which work is to be performed, except as provided in paragraphs (b)(2)(iii)(C) and (b)(2)(iii)(E) of this section. The lock shall be attached so as to prevent persons from operating the disconnecting means unless they resort to undue force or the use of tools. a. On or about January 3, 2017, located at the Core Baker machine, a personnel change occurred during a lockout condition when in the process of removing a heating element. An employee's lock was removed from a 480 volt panel leaving a common electrician's lock that all electricians had a key to. The oncoming electrician relied on this lock for protection while other employees were able to access the electrical cabinet. The cabinet was not isolated during a lockout activity, thereby exposing the employee to an electrical shock hazard of 480 vac and/or 240 vac.
