29 CFR 1910.179(j)(3): Complete inspections of the cranes were not performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section, depending upon its activity, severity of service, and environment, or as specifically indicated below. These inspections did not include the requirements of paragraph (j)(2) of this section and in addition, the following items. Any deficiencies such as listed were not carefully examined and a determination made as to whether they constitute a safety hazard: a) Rail mill cranes: On or about March 4, 2016, and at times prior, crane operators and workers near the crane were exposed to crushing and struck-by hazards related to incomplete, ineffective, and/or undocumented periodic inspections that failed to identify significant problems including inoperable brakes, loose and missing equipment, improper electrical installations, and inadequate lubrication and/or adjustment of moving parts. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
