29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Employees were using damaged equipment, which had external defects and evidence of possible internal damage. The following equipment was damaged: a) In the dock area, a NAPA Nitro Air portable air compressors power cord was missing a ground pin on the plug. The cord was wrapped in electrical tape and had a damaged outer jacket where a repair/splice had been performed. The high pressure cutout switch was missing a cover exposing the employees to approximately 115 VAC. b) In the maintenance area/shop, a Maxx Air High Velocity Fan, approximately 43 inches in diameter, was missing a ground pin on the plug.
