29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: On or about June 11, 2014, employees were exposed to struck-by hazards in that the employer did not conduct monthly chain inspections with a certification records. a) 5-ton overhead hoist located on the west end in the valve shop. b) 5-ton overhead hoist located on the east end in the valve shop. c) 5-ton overhead hoist located on the east end in the machine shop. d) 2-ton overhead hoist located on the west end in the machine shop.
