Waste Water Treatment Plant - On or about May 2, 2014, an employee was working under the elevated forks of the of the Komatsu LP forklift, SN 135952, and its supported load to perform his work of welding and cutting on a lime bin, and being exposed to struck by hazards of the falling forks or load. American National Standards Institute B56.1-1969 Safety Standards for Powered Industrial Trucks, Part III, For the User, Section 603 B. Operating Rules and Practices - General States; "Do not allow anyone to stand or pass under the elevated portion of any truck, whether loaded or Empty." No abatement certification or documentation is required.
