The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about May 30, 2013, employees were exposed to an electrical hazard, while plugging an extension cord into 120VAC electrical receptacle which was not metallically connected to the conduit to provide a continuous bond/ground.
