29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. a) 1718 North 42nd Street, Philadelphia, PA- On December 18, 2012 employees erected a tubular steel frame scaffold on the front sidewalk. The employees performing the work were not trained by a competent person to recognize the hazards associated with the erection of the scaffold. The upper steel frame was 20 inches from the closest phase of the 13.2KV distribution service and the planked working level was 28 inches away and at the same level. Conditions noted on December 19, 2012. NOTE: Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
