29 CFR 1910.146(i)(3): The employer did not ensure that each attendant continuously maintained an accurate count of authorized entrants in the permit space and did not ensure that the means used to identify authorized entrants under 29 CFR 1910.146(f)(4) accurately identified who was in the permit space: Employees entered tank 242 under a confined space permit to perform maintenance and cleaning. No record was made when contractor employees entered the tank for work repairing the tank seals. The space had to be evacuated, due to high LEL levels, and these employees were not removed from the tank.
