29 CFR 1910.119(e)(3)(v): The process hazard analysis did not address facility siting: a) Ammonia Room - The door leading from the boiler room to the ammonia room was not self-closing per ANSI/ASHRAE Standard 15-2010 8.11.2. It was binding to the floor and had to be forcibly pulled closed. This was not addressed in the 2010 PHA. On or about 2/8/12. b) Ammonia Room, upper part of the east wall - The openings around an 8" water discharge pipe from the condensers to the freezers, and a branch water discharge pipe that passed through the wall to the adjacent production area through were not sealed per ANSI/ASHRAE Standard 15-2010 8.11.2. This was not addressed in the 2010 PHA. On or about 2/8/12. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
