29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: a) In the 13, 14 Battery Transfer Area Pad Valve Pit, on or about August 11, 2025 - The employer did not ensure that the knowledge and skills required for the safe application and usage of 13 Pad Valve energy controls were acquired by employees who cleaned the seat of 13 Pad Valve with water when cryogenically-processed coke oven gas was present in the valve and Underfire lines. This exposed employees washing the valve, as well as other employees working in or adjacent to the 13, 14 Battery Transfer Area to an explosion when 13 Pad Valve ruptured during washing and released cryogenically-processed coke oven gas from the Underfire lines.
