Per ISA - grouped w/ Citation ID: 1-5 29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group. On or about 25 June 2025, employees of Gilster-Mary Lee Corporation, located at 615 Old St. Marys Road in Perryville, Missouri, were exposed to mechanical, atmospheric, and thermal hazards while working on or inside an industrial oven. A group of employees did not utilize procedures for lockout/tagout that afforded the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device while working on Sugar Coat 1 Oven.
