29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): A. Secondary Area: On, or about, July 1, 2025, safeguarding was not provided on the rolling mill that was used to re-size brass wire. B. Assembly, Ball Production Area: On, or about, July 1, 2025, the two hand controls provided on Machine #11, a RMT pneumatic press, had an anti-tie down relay that only worked for the right control, not for the left control. C. Assembly, Solid Race Area: On, or about, July 1, 2025, the two hand controls provided on Machine #19, a Greenerd hydraulic press, were not provided with rings, shrouds or other effective means to prevent unintended actuation of the controls. D. Assembly, Solid Race Area: On, or about, July 1, 2025, the two hand controls provided on the Blue Denison hydraulic press, brass tag 18932, were not provided with rings, shrouds or other effective means to prevent unintended actuation of the controls. E. Assembly, Solid Race: On, or about, July 1, 2025, the gap left under the access doors to the point of operation for the Capping Machine, brass tag 30926, exceeded established industry guidelines. Employees could reach under the guard.
