29 CFR 1910.305(b)(2)(i):Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: (a)National Food NW LLC, at 25351 National Avenue, Plankinton, SD 57368: On or about and at times prior to February 21, 2025, the employer did not ensure that damaged electrical equipment was repaired/replaced in that an outlet cover plate on the back of the wash station was not secured. This condition exposed employees to shock and electrocution hazards associated with the unsecured cover plate.
