29 CFR 1910.219(c)(4)(i): Unguarded rotating shaft ends projected more than one-half the diameter of the shaft: Employees engaged in grain elevator activities at the worksite located at N 1031 Topping Ave, Kansas City, Missouri 64120 were exposed to caught in and pinch point hazards. The employer did not ensure that employees were protected from unguarded rotating shafts when operating and working around trippers.
