29 CFR 1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Hammond's Candies, 5735 N Washington Street, Denver CO 80216: On or about January 24, 2025, employees previously authorized to operate powered industrial vehicles/PIVs (Crown Fork Lifts 5200 Series (Serial #9A186785)) at the site were not retrained and/or their ability to safely operate PIVs evaluated every three years as required thus potentially exposing the operator(s) and others working in the immediate area, to crushing and/or struck-by hazards.
