29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft ends did not present a smooth edge and end and projected more than one-half the diameter of the shaft: (a) Shear and Press Area - The 1.5-inch diameter horizontal drive shaft, between the three strands of chains on the chain conveyor at the rear of the Pacific hydraulic shear, model 600 G12, serial number S11176, had an unguarded end with a keyway and that projected 4.75 inches, exposing employees to caught-in and entanglement hazards, on or about May 6, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
