29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question: a) Worksite - On or about April 16, 2024, employees erected an Alum-A-Pole pump jack scaffold with the employees not being trained on scaffolding by a competent person to recognize hazards associated with the type of scaffold being used to include employees being exposed to fall hazards of approximately 29 feet and electrocution hazards due to working within the minimum approach distance when working near an uninsulated power line with voltage of 13.2kV phase to phase and 7.62kV phase to ground.
