29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: a.) PJ Murphy Forest Products Corp., - on or about 3/12/2024 and at times prior thereto: The company had not filled out permits prior to entry into confined spaces. Areas included, but were not limited to, drum roller dryer, hopper with auger inside, shaker/dust system, and boiler. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
