29 CFR 1904.7(b)(3): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent. Integra-Cast: Shell Room - In the following instance the employer did not properly record injuries/illnesses that resulted in days away from work in calendar year 2023: a) Case #1 - On or about November 09, 2023, and sometime thereafter, the fingers of an employee were amputated after getting caught in between a running belt of a rotating Mixing Flurry Drum that amputated two of his right fingers. This resulted in approximately 104 days away from work. The employer failed to enter the correct date and number of calendar days in the "Date of Injury or Onset of Illness" column (D) and the "Days Away from Work" column (K), respectively, when the date of injury provided was January 27, 2023, with 17 days away from work and 30 restricted days. Abatement Certification and Documentation is required.
