29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: At the worksite located at 1315 North 2nd Street Clinton, MO: On or prior to January 29, 2024, employees were exposed to respiratory, atmospheric, and explosive hazards. Employees worked at a grain elevator with twelve grain bins and two receiving pits which were not identified as permit-required confined spaces via signage. This occurred for at least twelve identified spaces: a) The receiving pit located near the office. b) The receiving pit located near the 100,000 bushel grain bins. c) The northern most 50,000 bushel grain bin. d) The 50,000 bushel grain bin located just north of the office. e) The 50,000 bushel grain bin located just south of the office. f) The third most southern of the office 50,000 bushel grain bin. g) The second most southern of the office 50,000 bushel grain bin. h) The southern most 50,000 bushel grain bin. i) The northern 100,000 bushel grain bin. j) The southern 100,000 bushel grain bin. k) The overhead bin located above the dumping and receiving area by the office. l) The overhead bin located by the dumping and receiving area by the 100,000 bushel grain bins.
