29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about May 3, 2023, the employer did not ensure that the tongue guard on the Dewalt bench grinder was adjusted within maximum opening of one- fourth of an inch from the grinding wheel. The adjustable tongue guard was adjusted beyond one-fourth inch of the wheel exposing employees to stuck by hazards during wheel failure.
