Text For Citation: 01 Item/Group: 005B Hazard:
29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a) Building 3, On or about January 27, 2023, The employer did not establish electrically safe work practices and policies to ensure only qualified personnel were accessing the live parts of the Gordon Brake Monitor for Machine #: 303 Make: Mitek Roof Tracker Model: 63826-501-460v Serial Number: 06007273320.1A, and that the practices included the appropriate level of personal protective equipment and testing equipment. Employees were making adjustments and resetting the brake monitor while energized with live electrical current greater than 50volt/AC, employees were exposed to electrical hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
