1926.1203(c) The employer, having identified, or received notice of, a permit space and having not authorized employees it directs to work in that space, did not take effective measures to prevent those employees from entering that permit space, in addition to complying with all other applicable requirements of this standard. 29 CFR 1926.1205(a): Before entry was authorized, the employer did not document the completion of measures required by paragraph 1926.1204(c) of this standard by preparing an entry permit: On and about August 19, 2022, the employer failed to take effective measures to prevent its employees from entering a permit space, in that the president of the company entered a sanitary sewer.
