29 CFR 1910.132(d)(1)(i): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE): 165 Pioneer Dr, Leominster MA: On or about 8/3/22, the employer had not adequately assessed the workplace to determine when the use of fire resistant clothing was necessary. Employees working in the blend-on-demand room, and other areas, were exposed to fire and explosion hazards while manufacturing windshield washer fluid using process safety management quantities of methanol.
