Text For Citation: 01 Item/Group: 005B Hazard:
29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Press area - On or about February 28, 2022, employees were working on the Press where the aggregate passes through an auger before being extruded into the mold, the augers guards had been removed without being locked out or procedures being developed, exposing employees to a caught in hazard. b. Blast - On or about February 28, 2022, employees were observed walking on a conveyor to clean up a broken paver without locking out the conveyor, exposing the employees to fall and struck by hazards. c. Blast - On or about January 20, 2022, employees were required to enter into the blast equipment to clear jams in the auger without the equipment being locked out, exposing the employees to a caught in hazard. d. Press Area - On or about February 28, 2022, employees were exposed to a caught in hazard while conducting change over and set up. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
