29 CFR 1910.212(a)(2): Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. a) On or about December 29, 2021, with respect to an establishment located at 1250 Glory Road, Green Bay, Wisconsin, employees were exposed to potential caught-in hazards when the guard doors to the power transmission sections of the James Hunter Machine Company, Model 360 BFS Openers on Line #1 were not affixed so as to prevent easy access to moving machine parts by employees. b) On or about December 29, 2021, with respect to an establishment located at 1250 Glory Road, Green Bay, Wisconsin, employees were exposed to potential caught-in and struck-by hazards when access panels to the rear section of the James Hunter Machine Company, Model 360 BFS Openers on Line #1 were not affixed so as to prevent easy access to moving machine parts by employees. c) On or about December 29, 2021, with respect to an establishment located at 1250 Glory Road, Green Bay, Wisconsin, employees were exposed to potential caught-in and struck-by hazards when access panels on the side of the Trutzschler Type FBK/V Chute Feed on Line #1 were not affixed so as to prevent easy access to moving machine parts by employees. e) On or about January 10, 2022, with respect to an establishment located at 1330 Glory Road, Green Bay, Wisconsin, employees were exposed to potential caught-in and struck-by hazards when access doors beneath the Line #3 Carding Machines were affixed with tamper seals. A cutter was also positioned on a lanyard directly beneath the lock for quicker removal of the tag. f) On or about January 19, 2022, with respect to an establishment located at 1330 Glory Road, Green Bay, Wisconsin, employees were exposed to potential caught-in and struck-by hazards when access doors beneath the Line #3 Carding Machines were were not affixed so as to prevent easy access to moving machine parts by employees.
