29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made: (a) Lexmark International Inc. at 6555 Monarch Road in Longmont, CO: On and before July 2, 2021, the employer did not ensure employees were safe from fire and electrical hazards in that the cord on the Hosokawa Micron Machine was torn and covered with tape, and the fitting it was attached to was too large to adequately secure the cord in place.
