29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: On or about March 11, 2021 at an inclined trash conveyor of the metal sorting line, the employer did not ensure that the inclined trash conveyor was turned off or shut down when an employee cleaned debris from the top roller drum of the conveyor. This exposed the employee to caught-by hazards.
