29 CFR 1910.178(m)(5)(iii):Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load lowered: a) Shop: On or about, January 27, 2021 and at times prior, an employee was exposed to a crush-by hazard, in that the forklift operator dismounted the seat of a Sunbelt Rentals telescoping forklift to perform welding to the sheet metal and did not lower the load.
