29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a)C & C Construction, Inc. at 82 Burlington Northern Santa Fe Railway, Hazel SD 57242: On or about and at times prior to May 22, 2020, the employer did not ensure that employees working off an elevated work platform supported by a Lull, Model 844C-42 Rough Terrain Forklift, were trained to recognize the hazards associated with working from heights on the platform. This condition exposed employees to fall hazards up to forty feet.
