29 CFR 1926.1211(c)(1): Each authorized entrant did not use a chest or full body harness, with a retrieval line: a) Bedrock Earthworks located at 5015 Big Run South Road Grove City, OH 43123: On and preceding January 27, 2020, the employer did not ensure that retrieval systems or methods were used when they entered a sewer. A crew was performing work that required entry into a confined space. There was a mechanical tripod on-site but it was not located at the manhole being entered nor was any other form of retrieval line connected to the entrant.
