29 CFR 1910.217(b)(14)(ii): The brake monitor was not installed on the press such that it indicated when the performance of the braking system had deteriorated to the extent described in paragraph (b)(14)(i) of this section: Instances including, but not limited to, the following: b) Job site, 33 McGovern Road, Crescent, PA: On or about November 23, 2019, during hands in die production activities on the Warco part revolution press number 2, serial number 5346, located in building 4, the employer did not ensure that the brake monitor system was installed in such a manner that it indicated when the safety distance for the 2 hand controls used for machine guarding had deteriorated to an unsafe limit.
