29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) On or about 09/27/2019 - Fuel House (a.k.a. Teepee), Boiler operators did not affix a lock on the three main electrical disconnects on the exterior of the silo to secure hazardous energy, exposing employees to caught-in and amputation hazards.
