29 CFR 1926.502(d)(1): Personal fall arrest system connectors were not drop forged, pressed or formed steel, or made of equivalent materials: a. On or about March 4, 2019, at the top of west gun barrel tank, no connector that was drop forged, pressed or formed steel, or made of equivalent material was used between a fall protection harness and a polyester sling used as a fall protection lanyard. A polyester sling end loop ran through the dee-ring of the fall protection harness to directly connect the fall protection harness to the sling. The fall protection harness and sling were worn by an employee for personal fall arrest when working from the tank top which exposed the employee to falls greater than 6 feet to a lower level.
