29 CFR 1910.305(h)(6): Strain relief was not provided at connections and terminations. On or about February 19, 2019, and in times prior thereto, 120 volt fourplex EP-3 B-23 had a strain relief device that was not effective, exposing workers to an electrical hazard, located in the production area.
