29 CFR 1910.272(g)(3): An observer, equipped to provide assistance, was not stationed outside the bin, silo, or tank being entered by an employee. Communications (visual, voice, or signal line) were not maintained between the observer and employee entering the bin, silo, or tank: (a) Rice Storage Bin#14 - On or about February 14, 2019 the employer did not ensure an observer was present at all times outside of the grain bin while employees were performing work inside the bin, exposing employees to caught-in and caught between hazards.
