29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: 83 Burlington St, Woburn, MA On about October 17, 2018, employees were exposed to electrical shock hazards while using an extension cord that had a cut in the outer protective casing exposing the live conductor wires.
