29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: A) Autoplas Division, Windsor Mold Group, located at 560 Goodrich Road, Bellevue, OH 44811: On or about August 20, 2018, the employer did not ensure that electrical safety-related work practices were utilized to protect employees from electrical hazards such as electrocution, electrical shock, and arc flash/blast. Maintenance employees conducted activities such as, but not limited to, troubleshooting and checking part number for the disconnect on the electrical panel for the hydraulic 1000 Ton Demag Press # 13 (Model: Titan 900; Serial # 0508). B) Autoplas Division, Windsor Mold Group, located at 560 Goodrich Road, Bellevue, OH 44811: On or about September 11, 2018, the employer did not ensure that electrical safety-related work practices were utilized to protect employees from electrical hazards such as electrocution, electrical shock, and arc flash/blast. Maintenance employees conducted activities such as, but not limited to, troubleshooting, cleaning, and replacing heater bands on the hydraulic 1000 Ton Demag Press # 13 (Model: Titan 900; Serial # 0508).
