29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Location: Prosys Room, palletizer and shrink wrap machines Lock out / tag out procedures for the shrink wrap machine are inadequate. An employee was injured while performing maintenance work on the palletizer. Instance A: The procedure does not include procedures for 'troubleshooting" the machine when pneumatic power to the 'arms' must remain 'on'. Instance B: When pneumatic power can be de-energized, the procedure does not call for locking out the valve. Instance C: The procedure does not call for verifying that electric power to the machine is 'off'. Instance D: The procedures do not adequately address de-energization of the shrink wrap machine.
