29 CFR 1910.217(b)(4)(i): Pedal mechanism(s) on mechanical power press(es) full revolution clutches were not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedals: Employees were exposed to a caught by hazard while working with mechanical power presses whose pedal was not protected to prevent unintended operation including but not limited to: a. On or about 5/22/2018, at the estate window department of CGI Windows and Doors Inc., 3780 West 104th St, Hialeah, FL. An employee was exposed to a caught by hazard while working with a mechanical power press (i.e. Rouselle Model No. 2, Serial No. 24068) whose pedal was not guarded. b. On or about 5/22/2018, at the estate window department of CGI Windows and Doors Inc., 3780 West 104th St, Hialeah, FL. An employee was exposed to a caught by hazard while working with a mechanical power press (Rouselle, Model No.3, Serial No. not available) whose pedal was not guarded. c. On or about 5/22/2018, at the designer department of CGI Windows and Doors Inc., 3780 West 104th St, Hialeah, FL.
