29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made: The employer is failing to protect employees from electrical shock hazards. This was most recently documented on 06/07/17 at the establishment located at 515 S. 4th Street in Osage City, Kansas. a) Employees engaged in production activities, using the #105 Cincinnati Milacron injection mold press, in the production area were exposed to electrical shock hazards in that the flexible cord supplying power to the press was not equipped with a ground pin. b) Employees in the Maintenance Shop area were exposed to electrical shock hazards while operating the horizontal band saw machine. The cord supplying power was being used with damaged exposed wiring.
